Format for Standard Forms

I have a few questions regarding the Technical Proposal - Standard Forms.

1. Is there a softcopy available on line? Where can we download these standard forms (i.e. TPF1-10 and FPF 1-6) if it is available online?

2. The forms are in Times New Roman font. Are bidders allowed to revise/change or use another font and font size?

Try visit the website of the GPPB and you can download the softcopy of the PBDs, specifically for Consulting Services. http://www.gppb.gov.ph/downloadables/default.asp

There is no specific prohibition, so long as the contents are readable, eay to understand, they may. They could even use a typewriter or a ballpen. It is important that you read the Preface of the PBDs so you are properly guided in using the same.

Good AM!

ano ang pwede naming i-issue sa sole bidder na declare eligible nung eligibility screening at pasado din sa technical requirements pero nung ma open ang bid nya nglumpsum bid sya instead na per item basis na nakaindicate sa bid docs. hindi namn pwede ang notice of post disqualification kasi hindi namn say na declare na winner dahil sa technicality (ie lumpsum instead of per item). hindi rin namn notice of ineligibility dahil eligible namn. ano dapat naming gawin?

nette, your BAC can simply sent a notice of post-disqualification to the Bidder for not follwing instruction pursuant to the provisions of your PBD's but, i think you have to evaluate the Bid first using the GPPB format and consquently issue the notice of post disqualification.

nette, the conduct of Bid Evaluation/Post Qualification under Sec. 32 & 34 is not only limited between two or more bidders for comparison of their bid documents, this can be done to a sole/single bidder too.

You are correct in precluding that this is not a case of Post Disqualification. Instead, this is a disqualification of a bidder after the Opening of Bids. You only proceed to Post Qualification/Post Disqualification if the bidder is rated PASSED after opening all the sealed bid envelopes and you have determined the ranking of those PASSED bidders according to Lowest Calculated Bid (LCB) or Highest Rated Bid (HRB) as the case may be.

Clearly, since the lone bidder is disqualified, you have to declare a failure of bidding. Very Happy

Sir/Madam, based on clause 19.2 of the procurement of goods third edition October 2009, are the prescribe forms are mandatory to use or we can modify it as long as it is the same and all the information’s are there. Is the list of on going and completed projects forms are mandatory to use? What if the information we have is not sufficient in the prescribe form? can we make ours? Is that subject for disqualification?

Clause 19.1 pertains to VIII bidding forms which includes bid form,contract agreement form, omnibus sworn statement and bank guarantee form and advance payment forms.
not the SAMPLE forms issued by the GPPB.

The SAMPLE forms issued by the GPPB is only samples which you could format as you please taking into considerations the current provisions of the IRR.

The GPPB does not yet issue revised sample forms.. but you can use the old sample forms..

Sir criscabuenos, while waiting for the forms to be downloaded, it is best if you ask from national government agencies in your locality. When you do not know, It is good to ask questions my friend.

this has reference to the Instruction to Bidders form. are all the things found in the PBD Goods - ITB table of contents included in the instruction for bidders forms? or can we edit the ITB and include only those things that is important for the bidders to know? some clarification please...thanks a lot.

If you want to include or not include provisions in the ITB which you believe is/are applicable/not applicable to your procurement, you can have them in the Bid Data Sheed (BDS). You will take note that the BDS guides you on what options to take.

Excerpts from Section II. Instructions to Bidders which is self explanatory.
Notes on the Instructions to Bidders

This section of the Bidding Documents provides the information necessary for Bidders to prepare responsive bids, in accordance with the requirements of the Procuring Entity. It also provides information on bid submission, opening, evaluation, and award of contract.

Section II contains provisions that are to be used unchanged. Section III consists of provisions that supplement, amend, or specify in detail, information or requirements included in Section II which are specific to each procurement.

Matters governing performance of the Bidder, payments, or those affecting the risks, rights, and obligations of the parties under the contract are not normally included in this section, but rather under Section IV. General Conditions of Contract, and/or Section V. Special Conditions of Contract. If duplication of a subject is inevitable in the other sections of the document prepared by the Procuring Entity, care must be exercised to avoid contradictions between clauses dealing with the same matter.

Sir Criscabuenos, we are in the same predicament and I hope the GPPB should fast track the issuance of new forms, but in the meantime, as posted by Sir Niwram, we can use the old forms.

Is there available new standard form for Purhcase Order , Purchase Request and other appurtenant forms used in processing procurement transactiions

As of press time, the Purchase Order Form and Purchase Requst form has not been changed yet.
