INVITATION TO APPLY FOR ELIGIBILITY AND TO BID

The MUNICIPALAITY OF DANAO , through its Bids and Awards Committee (BAC), invites suppliers/manufacturers/distributors/ contractors to apply for eligibility and to bid for the hereunder projects:

Name of Project : Purchase of Medicine Supplies and Laboratory Supplies for MHO
Location : LGU-Danao, Danao, Bohol
Brief Description : Medicines Supplies
Approved Budget
For the Contract (ABC) : P 90,572.25
Contract Duration : ____________________________

Prospective bidders should have experience in undertaking a similar project within the last (state number of years, minimum 2) with an amount of at least 50% of the proposed project for bidding, and have key personnel and equipment (owned, leased or under purchase) available for the prosecution of the project. The list of minimum equipment requirement is indicated in the Eligibility Forms. The Eligibility Check/Screening as well as the Preliminary Examination of Bids shall use non-discretionary “pass/fail criteria. Post-qualification of the lowest calculated bid shall be conducted.

All particulars relative to Eligibility Statement and Screening, Bid Security, Performance Security, Pre-Bidding Conference(s), Evaluation of Bids, Post-Qualification and Award of Contract shall be governed by the pertinent provisions of R.A. 9184 and its Implementing rules and Regulation (IRR).

1. Issuance of Bid Documents October 6 – 14, 2011
2. Pre-Bid Conference Not Applicable
3. Submission of Letters of Intent and Application for
Eligibility (Single-stage bidding is used, delete this activity) October 6-14,2011
4. Opening of Bids October 14, 2011
5. Bid Evaluation October 17, 2011
6. Post-Qualification October 17, 2011
7. Notice of Awards October 18, 2011

Bid documents will be available only to prospective bidders upon payment of a non-refundable amount of Three Hundred Pesos Only (P 300,00 ) to the Municipality of Danao Cashier.

The MUNICIAPALITY OF DANAO assumes no responsibility whatsoever to compensate or indemnify bidders for any expenses incurred in the preparation of their bids.

agree with engrjhez...you can check with the standard forms and the procurement milestone for this particular opportunity....please look on downloadable PBDs of the GPPB website for further info...
