What are the documents need to be notarized?

I would like to know if there is a list of documents for bidding that needs to be notarized? Also, does CLC has to be notarized as well? Thanks

The law only requires two, namely

1. In the absence of a JVA, duly notarized statements from all the
potential joint venture partners stating that they will enter into and abide by
the provisions of the JVA in the instance that the bid is successful shall be
included in the bid. Section 23.1 b., 24.1 b.

2. The signatory is the duly authorized representative of the
prospective bidder, and granted full power and authority to do,
execute and perform any and all acts necessary and/or to
represent the prospective bidder in the bidding, with the duly
notarized Secretary’s Certificate attesting to such fact, if the
prospective bidder is a corporation, partnership, cooperative, or
joint venture. Section 25.2

How about the list of on going and completed contracts? Does the requirement depends on the Agency? I noticed that each Agency differs

No need to notarized other documents except those that had been mentioned by Mam Regina and the Omnibus Sworn Statement but other documents needs to be validated during the post qualification stage in order to determine the authenticity of the documents.

We were disqualified during the Post Qualification because our CLC is not notarized same with the List of on-going and completed contracts.

I don't think there is a provision in the Revised IRR that those documents needs to be notarized. Maybe you could file an MR requesting clarification why there is a need to notarized those documents which are not required by law.

I agree that the Revised IRR does not say whether these documents are required to be notarized. I believe however that the procuring entity prescribed a format for all those documents, which were patterned after GPPB sample forms. GPPB sample forms indicate that CLC and list of ongoing contracts have to be acknowledged before a notary public.

There is only one: Omnibus Sworn Statement. It becomes two when the bidder is a JV.

CLC is a bank issued document. Its notarization depends on the requisite of such bank in validly issuing such instrument.

So if it is a JV, two Omnibus would be submitted also? I thought that only one is needed since there is an SPA that authorizes the signatory to sign and submit the documents. Thanks this really helps
