Dear Contractor,

1. The Municipality of Siocon hereby requests you to submit a quotation for the
following works:

Construction of Jolamipaman ARC Warehouse
Municipality of Siocon,
Province of Zamboanga del Norte

To assist in the preparation of your price quotation, the necessary specifications, bill of quantities and drawings, form for submitting the quotation and a draft contract form are enclosed.

2. You shall submit one original of the Price Quotation with the Form of Quotation, and
clearly marked “Original”. In addition, you shall also submit one copy marked as “COPY”.

3. Your quotation in the attached format should be sealed in an envelope and addressed to and delivered at the following address:

4. You must have experience as a prime contractor in the construction of at least one work
of the nature and complexity equivalent to the works included in this Request for Quotation, over the last three years and provide evidence of financial resources to successfully complete the works.

5. Each bidder shall submit only one quotation, either individually, or as a partner in a joint
venture. All quotations submitted in violation of this rule shall be rejected. Partners in a joint venture shall be jointly and severally liable for the execution of the contract. In the case of a joint venture, the lead partner should demonstrate work experience under paragraph 4 above. Financial resources of all the partners in the joint venture will be combined to determine adequacy of resources.

6. In evaluating the quotations, the Employer will determine for each proposal the evaluated price by adjusting the price quotation by making any correction for any arithmetical errors as follows:

a) where there is a discrepancy between amounts in figures and in words, the amount in words will govern;
b) where is a discrepancy between the unit rate and the line item total resulting from multiplying the unit rate by the quantity, the unit rate as quoted will govern;
c) if a Contractor refuses to accept the correction, his quotation will be rejected.

7. Your quotation shall be valid for a period of forty five (45) days from December 10, 2012

8. Your quotation in English language shall be for the whole works and based on the unit and total price indicated in the Bill of Quantities for a fixed unit rate contract. Currency of quoted prices and payment shall be in Philippine Peso. The quotation shall include all duties, local taxes and other levies payable by the contractor in accordance with the local laws.

9. The Employer will award the contract to the Contractor whose quotation has been determined to be substantially responsive to this Request for Quotation and who has offered the lowest evaluated price quotation.

10. A Contractor, who withdraws his quotation during the validity period and/or refuses to
accept the award of a contract when and if awarded, will be excluded from the list of contractors for the project for two years, subject to paragraph 14.

11. The contract will be governed by the terms and conditions of the attached draft
Contract.

12. Your quotation (Form of Quotation and filled-in Section 2 “Bill of Quantities/Priced
Activity Schedule”) should be submitted by December 10, 2012 at 10:00 AM, and shall be accompanied by a bid security in a form stated below.The quotations shall be opened in public in the presence of contractors’ representatives who choose to attend, on December 10, 2012 at 10:00 AM same date as for bid submission at Municipal Mayor’s Office 2nd Floor MunicipalHall Building, Brgy. Poblacion, Municipality of Siocon, Zamboanga del Norte.

(a) Cash or cashier’s/manager’s check issued by a Universal or Commercial Bank. Two percent (2%)
(b) Bank draft/guarantee or irrevocable letter of credit issued by a Universal or Commercial Bank: Provided, however, that it shall be confirmed or authenticated by a Universal or Commercial Bank, if issued by a foreign bank.
(c) Surety bond callable upon demand issued by a surety or insurance company duly certified by the Insurance Commission as authorized to issue such security; and/or Five percent (5%)
(d) Any combination of the foregoing. Proportionate to share of form with respect to total amount of security

13. The Employer intends to apply funds from the Asian Development Bank (ADB) for eligible payments under the Contract resulting from this RFQ.

14. Under ADB’s Anti corruption Policy bidders shall observe the highest standard of ethics
during the procurement and execution of such contracts. ADB will reject a proposal for award, and will impose sanctions on parties involved, if it determines that the bidder recommended for award or any other party, has engaged in corrupt, fraudulent, collusive, or coercive practices in competing for, or in executing, the Contract.
