I got confused.

If it is meant to be a canvass report/form, I suggest that it be simplified to indicate only the necessary data for the procuring entity's guidance. Granting that canvassing is supposed to simplify the process (for alternative methods), maybe it would be advisable to simplify the form, as well.

Yes, it is supposed to be a simplified CANVASS-and-AWARD FORM. The BAC Resolution need not be attached. However, the major concern here is "where's the BAC?". The BAC is actually portrayed by a smaller number of members. Instead of five (5) BAC signatories plus the HOPE, and in our case, plus the TWG and the Secretariat (that's eight all in all), the signatories came down to four (4). The BAC Resolution for the purpose shall be mentioned, with the approving body to be delegated to the department head concerned.

The actual canvass is summarized in the form above as well as the recommendation for award. This should fit right subject for "blessing in fire" here. Smile

In my opinion, it may complicate things instead of simplifying the process because BAC's signatures have been omitted. We would rather use the abstract of bids (as read / calculated) and then a separate resolution issued to declare winning bidder and for an award.
You might run into trouble with COA. Please refer to COA Circular 2009-001 issues Feb. 12, 2009.

The form above is not intended for public bidding but to selected alternative methods of procurement, especially shopping. Of course, this is still to be supported by the supplier's legal documents. Thanks for your valued input. Smile

For us GOCC we use the Standard Form Number: SF-GOOD-60 with certain modification in the General condition to bidders

1. All Entries must be typewritten
2. Delivery period within _____ calendar days
3. Warranty shall be for a period of period six(6) months for supplies & materials, one(1) year for equipment from date of acceptance by the procuring entity
4. Price validity shall be for a period of _____ calendar days
5. G-EPS Registration Certificate shall be attached upon submission of the quotation
6. Bidders shall submit original brochures showing certifications of the product being offered if applicable
7. Payment Terms NO COD
8. The Bid must be COMPLETE, Bids not addressing or providing all of the required items shall be considered non-responsive and, thus, automatically disqualified
9. Any interlineations, erasures, or overwriting shall be valid only if they are signed or initialed by the person signing the Bid.
10.Award shall be made using the PASS/FAIL criteria

good day.... to have transparency our request for qoutation have three members (one from the accounting office, budget office and the general services office) so that every transactions are well coordinated.....the bac members will then have an abstract of canvass/qoutations which they signed and recommend for award to the lowest obtainable price

We appreciate your post although it is out of the topic. Your suggestion is good. As a matter of fact, government employees also are rated with the PES and the PMS-OPES as standardized by the Civil Service Commission. Im afraid however, that some of the entries in your proposed form does not fit or does not apply for government employees. Very Happy

Good evening! The wisdom behind the suggested form(at) for alternative mode of procurement (shopping for that matter), is acceptable, with a little more refinements. As mentioned, that would ease the work of the Secretariat.

However, with the present set up, the form(at) may run contrary to RA 9184 and Auditing rules and regulations as it does not bear the approval of the BAC perse. To opine further, canvassers will be so much armed with power were shopping is concerned.
